Psychotherapy is remarkable but it is not clear how it works.  Although there are many theoretical models for how to perform psychotherapy.  The Revised Contextual Model is a meta-model that explains how all approaches to psychotherapy are effective and also provides insight to all healing practices, including medicine and indigenous/cultural healing methods.  The most important factor in psychotherapy is the therapists—some therapists are consistently more effective than others, regardless of treatment being used.  Knowledge of the characteristics and actions of effective therapist leads methods for quality improvement.  As is the case of experts in all fields, therapists can participate in deliberate practice of the skills displayed by effective therapists to become more effective.
